Does anyone know how to get the proof of life form? Is there a way to download it and send it back to the US?

Sometimes you can send it to the FBU at the US Embassy. Yes, you can download the form, print it, fill it out, and send it. Check with the FBU on whether to send it to them or to the US. Sometimes US customs won't let them into the country

Download and print it from the US Embassy website, fill it out, and send it by local courier to the US Embassy in Manila. The mailing address is listed there. Quick and simple.
