No, foreigners who only have Balikbayan privileges are not eligible to enroll in PhilHealth under their own name. According to PhilHealth Circular 2017-0003, foreigners can enroll in the informal economy category only if they have a long-term stay visa such as the SRRV (Special Resident Retiree's Visa), or a valid Alien Certificate of Registration (ACR I-Card) tied to a long-term immigrant, non-immigrant, or work visa.

Submit a copy of your marriage certificate, a copy of your passport, a copy of your Balikbayan stamp, and a 1x1 photo. The cost is 17,000 pesos a year.

As others said, you can enroll in PhilHealth with a voluntary ACR card, and that's what I did. Just use it as a supplement to other insurance, though. The coverage isn't that broad, but it's good to have.

The Yakap program is also worth looking into.
