We also found this out when we first came here. It's obvious that there's no proper free secondary healthcare in this country. We looked at a bunch of health insurance products and they were ridiculous. Total rip-off. So we put 100 pounds into a bank account every month. The money we'd need if something happens is sitting there, and if we don't use it, we go on a nice holiday in Europe with it 🤷‍♂️

I heard Pacific Cross is also an option in the Philippines. A decent agent is Michael Onstead.

Try Allianz Well. It covers up to 100 million pesos worldwide, and all the big hospitals admit you without cash up front. The other products have coverage that's way too small.
